Important Notice: Change to Our Clinical IT System

We will be upgrading our clinical IT system between 07/04/2026 and 17/04/2026.

During this period, there may be some temporary disruption to certain services while we safely transfer and update patient records. We appreciate your patience and understanding while this essential work is carried out to improve our services.

What You Need to Do

Check Your Contact Details

Please ensure that your contact information, including your email address and telephone number, is up to date.

If your details have changed, please inform our reception team as soon as possible.

Order Repeat Prescriptions in Advance

If you are likely to require repeat medication during the changeover period, please ensure that your prescription is ordered no later than 01/04/2026.

We cannot guarantee the immediate processing of repeat prescriptions during the system transition, so ordering in advance will help prevent any disruption to your medication.

We apologise for any inconvenience this may cause and thank you for your continued cooperation and understanding.

New Patients

We accept new patients who live in Baglan, Sandfields, Aberavon, Port Talbot, Taibach, Margam, Cwmavon and Pontrhydyfen. 

You will need to complete the GMS1W Patient Registration Form and the relevant questionnaire.

If you do not have access to a printer, all forms can be collected from reception

The GMS1W Registration Form MUST be signed in the bottom left corner of the front page. We cannot accept it unsigned.

  • Both forms can be dropped off at reception. GMS1W form MUST be signed.
  • Please provide two forms of identification on registration with the practice. Please bring one from each category below
    • Category 1: Proof of identity: birth certificate, marriage certificate, photo ID, passport, drivers’ licence.
    • Category 2: Proof of address: Bank statement, utility bill, rent/lease agreement, mortgage letter.
  • A copy of repeat prescription slip for your medications is required for our records.
  • Your NHS number: This is a 10-digit number, like 485 777 3456. You can find your NHS number on most documents or letters sent to you by the NHS such as prescriptions, hospital letters, appointments letters. If you are still not sure we would advise you to contact your previous practice for your unique NHS number.

Once the above documents and completed forms have been provided to the practice you may be offered a registration health check appointment.
